Five companies are cited for the death of an Amazon.com worker in Avenel

     Federal labor officials have cited five companies connected to operations at an Amazon packaging facility in Avenel for serious violations following the death of a worker.  The U.S. Department of Labor's Occupational Safety and Health Administration said yesterday that the contractor responsible for operating the facility Woodbridge facility and four temporary staffing agencies were cited in the death of temporary worker Ronald Smith, who died from injuries sustained in December after he was caught in between a conveyor system and crushed while performing sorting operations.  OSHA cited Pittsburgh-based contractor Genco for failing to certify that a hazard assessment of the facility had been conducted before employees were assigned.
